Porezna uprava (Tax Administration)

OIB application — Form OIB-1 (personal identification number)

OIB-1 form can be submitted in person at any Porezna uprava office or via the e-Porezna portal for holders of a valid eID.

Address registration — prijava/odjava boravišta

Must be done at a MUP office within 3 days of taking up residency. Landlord must be present or provide a signed consent. Bring rental contract.

HZZO (Croatian Health Insurance Fund)

HZZO health insurance enrollment — Form T-1

T-1 form for employees is submitted by the employer. Self-employed submit directly to HZZO.

Issued from the original country's equivalent authority (not Croatian), then apostilled. Croatia does not issue this for foreign nationals. Request the equivalent from your country's Ministry of Justice.

Pet passport is issued by an accredited (official) vet in your origin country. Contains microchip number, rabies vaccination record, and (if required) titer test result.

Business registration — paušalni obrt or d.o.o.

Paušalni obrt (sole trader under the flat-rate cap) is the simplest structure for freelancers. FINA processes the registration. OIB is required before you can register.
